REFERRAL PROCESS

Patients may be self-referred, referred by their physicians, other concerned persons, agencies, or their employer.

1. 24-hour answering services are available to provide for phone contact. If the emergency is a life-threatening emergency call 911 immediately or check-in to your nearest emergency room.

2. On a routine basis, patients are seen on an outpatient basis for evaluation and treatment planning.

3. Reports will be sent to the referral source at the time of evaluation, during the course of treatment, and at discharge. This will always be with the person’s express permission (following HIPAA guidelines).

4. For continuity of care, appropriate information will be shared with the personal physician as requested.

 

Patients will be referred to their physician for continuing care. HIPAA guidelines are followed.
